I'm not sure DoomMQ works anymore. Even on my setup I haven't managed to get it to work anymore, which is a real shame imho. I recommend using MQDab....

Yes. If you can find them in "Assault Mecha" as a player, as the shop itself I believe has been removed.

After fiddling around with it, I've found a better method.
1. Open up BO2 and Inject | It's best to inject a second or two after you click "online".

Having issues with this. I watched max prestige clip and I'm still having issues with "steam connect" error.
EDIT: I've even reset my stats, and still having issues.

1. Open up Fiddler.
2. Attach to MQ dab or DoomMQ or trainer.
3. Go to ANY Mech Shop. Check Fiddler for cf-loadmechshop.swf

I didn't make any trainers as I don't know how to. I just managed to edit some code from some old SWF loaders. You can still utilize Doom MQ's SWFs, just slot them into MQdab's swf folder, restart...

MQDab still works, still not sure why DoomMQ stopped working. It was my favorite trainer. Still, you can take a lot of the same SWFs and use them in MQDab.

You cannot add novagems, no. I have tried all the different methods, including modifying Ballyhoo's rewards to be like 10000k Novagems, it doesn't stick.

I just tested it with the included trainer and it works for me. Do you have Flash installed?

I'm not sure exactly how to do so. I imagine there is a way, but I think there's a better overall method.
