How is it calculated, and is it accurate?

Whereever the server is hosted from.

i believe it is

Calculated by adding up your time online?

Can be easily tampered with though

As long as your logged in on mpgh, your time goes up.

Server sided. I was thinking more along the lines of a cron script that visited a page every 30 minutes

I left my Killian account logged in for about 2 days, and it counted as online time, even though I was afk for most of it.

Yeah, it'll count your idle time also. I've done that too.
