Inject "MW2 Lobby Tools.dll" into MW2 with your favorite injector (recommended: Rullez Injector)
Press F1 to open the console. (Note: the console displays no output, but a lot of dvars can still be set. (r_fullbright 1, r_fog 0, cg_thirdperson, etc.)
Press F2 to open game setup menu
Press F3 to force start the game (Note: all party players will join the spectator team. Change team using F5)
Press F4 to toggle force host
Press F5 to toggle privatematch/publicmatch. (Note: switching to private match allows you to change team. I recommend changing team before players start or after all players have connected (otherwise random joiners will join the spectator team))
Notes:
Make sure you have open NAT (Enable UPnP or open port 28960 if your NAT is closed (If you tried this and it still shows NAT Strict, it could be that it is actually open but just doesn't show right.)
VAC:
It's a user32.dll hook and an injection. Uses calls to in game functions. Detected? I really don't know. Doesn't matter anyway since you can play while being banned...
Well not all commands. Stuff like map mp_afghan or kick does not work. But most dvars do work.

Can't seem to use Yano's wallhack with this, the aimbot function works though.

Force host seem to not work sometimes when you have a party.
I can't seem to get the game setup to work? I press f2 and the menu pops up but any changes I make don't affect the game. Do I have to be host for it to work?

Yes, you need to be the host obviously to change the lobby. You also need open nat type to ever be the host
